After many years of providing psychiatric care to the South Jersey and Greater Philadelphia communities, Centra closed its doors on November 22nd, 2025. We are deeply grateful for everyone who put their trust in us over the years.

- If you are requesting Centra medical records please follow these instructions:
- Use this sample to complete a blank release of information. (if you received psychotherapy also see this psychotherapy sample and fill out this blank release of information form)
- Take your complete release(s) and a copy of your driver's license and email to info@centrapc.com
- Once the completed request is received a request will be made to the holder of the medical records Morgan Records Management and a total page number will be determined.
- Once the total page number is determined you’ll receive an email with the total cost. The cost will be $1 per page up to $50 for patients and for third party requests it’ll include a $20 search fee and $1 per page with no cap.
- All payments must be received by check before records can be released.
- Checks can be mailed to Centra Medical Records at 1930 E. Marlton Pike Ste D19 Cherry Hill, NJ 08003.
- Once the check is received and deposited, the records will then be sent to an email or fax number
